Refund Policy
The school shall adhere to the following refund policy in the event of notification of withdrawal by the student or termination by the school prior to completion of the course or program. All refunds will be made within 30 days of the cancellation or termination date. Cancellation must be made in person or by certified mail. The refund policy follows South Carolina Commission on Higher Education Commission regulations. The student will only be considered dropped once the CDAA provided drop paperwork form is signed, dated, and received by CDAA. Poor attendance or non-attendance in a program, verbal indications of intention to drop provided to any CDAA related personnel or any other paperwork that has been submitted with or without an indication of an intention to drop will not substitute for notification of cancellation or dropping of a class.
Refund calculations are made based on the following conditions:
Within Three (3) Days of Signing Enrollment Agreement
All monies will be refunded if the applicant is not accepted by the school or if the applicant cancels or is terminated by the school within three
(3) business days of signing the Enrollment Agreement. Cancellation must be made in person or by certified mail.
Three (3) or More Days of Signing Enrollment Agreement, but Before Attending First Class
Should a student be terminated or request cancellation three (3) or more business days after signing the Enrollment Agreement, but before attending the first class, all monies paid, less the nonrefundable application fee of $100.00 and the cost of any materials that are not reusable or equipment not returned to the administration office, will be refunded.
If No Notification Made Prior to Program/Course Start Date
An applicant not requesting cancellation in writing by his/her specified program/course start date will be considered a student. If no cancellation request is made in writing prior to the start date of the program/course, and the student is absent for more than 4 hours of class without explanation, CDAA will then consider the student to have withdrawn from the program/course. All monies paid, less the nonrefundable application fee of $100.00 and the cost of any materials that are not reusable or equipment not returned to the administration office.
After attending the first class
If a student chooses to withdraw in writing or is terminated from this program/course for any reason after attending the first class, refunds are based on the proration of tuition and percentage of program completed at withdrawal, up until 60% of the program, less the nonrefundable application fee of $100.00 and the cost of any materials that are not reusable or equipment not returned to the administration office. The cancellation or termination date for refund computation purposes shall be the last date of actual attendance by the student.
Class Times/Location
Class times and locations vary according to CDAA location, course, and lecture/lab schedule. Students are issued location/class-specific schedule at orientation prior to the start of the selected course.
Completion
In order for a student to receive a Certificate of Completion, they must attend 90% of the course, pass the final exam with a 70 or above, and pass the overall course with a 70 or above.
Textbooks
Students will receive a textbook that they can use while they are enrolled in the program. Before the final exam is given, students in these classes must return the textbook they were issued. Textbooks are not to be kept by the students and students will be charged for the cost of the textbook if they do not return it to the school.
Computers
Students are encouraged to bring their personal computers to each class. These classes offer simulation software that the students can utilize to practice and gain more experience in a dental office. Students will be provided the simulation software during orientation and will be walked through how to install it on their computer. If a student is unable to procure a laptop for use during the class, CDAA will provide a laptop or desktop for the student to use while on campus.
Third-Party Loans
Third-party loans must be repaid according to the terms of the note even if the borrower does not complete his or her education, cannot get a job after completion of the program, or is dissatisfied with the education.
Other Terms and Conditions
A student may be terminated for creating a safety hazard to faculty or other students, disobedient or disrespectful behavior to faculty or other students, unsatisfactory academic progress, poor attendance, unprofessional conduct or behavior, failure to remit payments when due, cheating, falsifying records, breach of the Enrollment Agreement, entering any CDAA premises while under the influence of alcohol, drugs, or narcotics of any kind, carrying a concealed or potentially dangerous weapon, or harassment, including sexual harassment of any kind. A student that is terminated for any of these reasons is not subject to receive a refund.
CDAA cannot guarantee job placement or employment although we make every effort in working with our students to increase their chances of finding employment. CDAA does make no claim or guarantee that credit earned will transfer to another institution.
Student has been issued a copy of, and agrees to abide by or adhere to all additional policies and terms contained in, the Program/Course Listing and Information Catalog.
Hold Harmless Statement
CDAA and student acknowledge that there is some risk of accident or injury associated with use of equipment and other aspects of the course of study, including but not limited to direct care and contact of other students or clients at the clinical or training site. Student does hereby waive, release, and discharge CDAA of any and all liability and all claims for damages for death, personal injury, or property damage which I may have or which hereafter accrue to me as a result of participation in the program whether or not caused by negligence or fault of CDAA.
This release is intended to discharge the school, and its officers, employees, representative, volunteers, and agents from and against any and all liability arising out of or connected in any way with my participation in the training, internship/externship, hands-on activities, practice, or other activities.
Knowing risks exist, nevertheless, I hereby agree to assume those risks and to release and to hold harmless all persons or agencies mentioned above that might otherwise be liable to me or my heirs or assigns for damages. I further understand and agree that this waiver, release and assumption of risk is to be binding on my heirs and assigns. In addition, I give permission to receive, if necessary emergency medical services by authorized personnel, and that nay cost incurred as a result of such medical treatment will by my responsibility.
